Chemical exfoliation is a procedure where a chemical acidic agent of defined strength is applied to the skin to cause a controlled mild reaction up to a desired depth. The healing process which follows causes increased turnover of epidermal cells called keratinocytes, and stimulates the fibroblasts in the dermis to produce new collagen. These two effects lead to reduction of pigmentation due to rapid shedding of epidermal keratinocytes [called exfoliation], improvement of skin texture, colour, reduction of scars, improving the firmness of the skin and smoothening of fine wrinkles. The depth of penetration and the destruction of upper layers of the skin caused by the peel depends on the composition of the chemical, concentration and the duration of time the peel is applied. The depth of penetration also depends on the thickness of the skin, especially the outermost dead horny layer and amount of sebum (oil secretion) on the skin. Clients with thinner skin are usually more sensitive to peels. The benefits that are desired can be obtained by varying the composition of the peel, modifying the concentration of the chemical, and controlling the duration of exposure when the peel is applied. The benefits of chemical peels can be enhanced by using therapeutic skin medication to enhance a desired effect, and internal remedies to improve the integrity of the skin. Application of a peel causes “burn” effect which causes mild redness of the skin. This redness subsides in a few hours or up to a day. During this time, the patient must avoid exposure to direct sunlight. This can restrict the patient’s outdoor movement. This period is called “downtime”. Depending on the depth of penetration, peels are grouped as superficial, medium or strong peels. The doctor may advise the number of sessions (5-8) based on the skin type. The treatment is performed with a gap of 15 days between 2 sessions. Maintenance sessions can be taken every 30 days or as needed.
